Do you have a balance due? You can now pay your balance online! Please call us at 801.266.2485 to give us your email address, and we will send you your login information to access the Patient Portal. You may also register online by selecting the ‘Patient Portal’ button below and then ‘Register’.

No show fee – A no show fee will be billed for cancellations made within 24 hours of appointment:
Therapy services: $50.00
Psychological services: $100.00
*Please note that the above rates are not applicable to insurance billed services. If you have had services billed to your insurance company, the rate is contingent on contractual agreements that cannot be reduced if applied to deductibles, coinsurance, or out-of-pocket costs.

Please note that when you set up your initial appointment, we will make every attempt to verify your insurance. However, it is always a good idea to verify with your insurance plan directly before you give us a call. That way, you will know if it requires an authorization beforehand, and you can also learn about any co-payments, deductibles, exclusions, or limitations that may affect your treatment.

Ward Pay
For those of our clients who are LDS, we do accept payment from your local ward. Please note that you will have to set up payment with your bishop and get authorization first. A ‘release of healthcare information’ form will also need to be put on file. We can then work with both of you on payment.
